DOT/CDL Examinations

DOT/CDL PHYSICAL EXAMINATION

Step 1: Fill out a brief 1-page health history.  The form can be found on this page to download and fill out before your appointment.


Step 2: Complete your DOT physical exam with Dr. David Fisher, D.C. who is a certified medical examiner for the Federal Motor Carrier Safety Administration (FMCSA).

  • Review of brief medical history
  • Height, weight, blood pressure, pulse rate, range of motion, eyes and ears
  • Discuss special medical conditions
  • Urinalysis for basic procedure

Step 3: We complete your paperwork.  Photo ID required.

  • You get 2 copies of the DOT Medical Certificate to keep with you at all times while you are driving. 
  • You get a copy of the DOT physical form, one for your company if needed for insurance purposes.
  • We will keep a scan copy of your DOT physical and license on file at our office. 

Things to Plan in Advance:

If you have a medical condition you're going to need some paperwork:

  • Diabetic Condition: bring a list of medications and a note from your prescribing doctor stating your compliance.
  • High Blood Pressure: bring a list of medications and a note from your prescribing doctor stating your compliance.
  • Heart Condition: If you've had a heart attack or cardiovascular disease you'll need to bring a copy of your last stress test and work clearance from your doctor. 
  • Sleep Apnea: bring a copy of your latest sleep test results. 
  • Medications: list all the prescriptions medications you take, including strength and dosage. If you regularly take over-the-counter medications, such as antacids or allergy pills, list them too. 
  • Contact Information: have names and phone numbers of your doctors in case you need to call and have them fax missing information. 

The Day of the DOT/CDL Physical Exam:

  • Please download DOT/CDL Forms and have them completely filled out prior to your examination - this will save time. 
  • Remember to bring the medical records you've prepared in advance, including your list of medications. 
  • Remember to bring eye glasses and/or hearing aids.
  • Drink water. Come with a need to urinate.

Dr. Dave Fisher is now certified for drug screening, pre-employment testing, post accident, random, and return to duty urinalysis.
